Is there a best or #1 clinic/ staff in Costa Rica?

There are very good capable, honorable dentists and clinics in Costa Rica. But categorically there is no #1 or best facility. Any such claim is both misleading and false. Confirm with the CCDCR legal department( Fiscal) who establishes the rules and code of conduct to be followed by dentists and clinics. All dentists have been advised in their advertising, etc., to make no such claim. This false assertions will be defined as both unprofessional and unethical. No organization has ever made any such evaluation or survey of the dental community. If a clinic (s) makes a claim that they have a ‘world’ rating, be careful. No organization has ever evaluated clinics in every county in both hemispheres. Seek out the reputable top quality clinics that  exist in Costa Rica.

Do we need a visa and what is the time limit?

US citizens do not require an entry visa to Costa Rica (Canadian nationals should contact their embassy).

However, they must have a current valid passport and a return ticket to exit Costa Rica within 180 days. (Either to return to the US or to go to another country).
